Our hospice specialists are available upon request for a no-cost, no-obligation visit. LENITY HOSPICE will send a clinician to the home, hospital, assisted living facility or nursing home to make a clinical assessment and to answer any questions. Our goal is to respond as promptly as possible to make the admission process as smooth as possible.

  • What is hospice care?

    Hospice focuses and provides special care to improve the quality of life, in most cases, care is provided in the patient’s home. Hospice is not about giving up hope but rather a way to get the most appropriate and best quality care in the last phase of your life. This enables you to spend your final days in a familiar, comfortable environment, surrounded by your loved ones and supported by hospice staff. Hospice is traditionally an option for people whose life expectancy is six months or less. Hospice services are available to patients of any age, religion, race or illness. Hospice is covered under Medicare, Medi-Cal, most private insurance plans, HMO’s, and other managed care organizations.

  • The hospice team develops a care plan tailored to your individual need for pain management and symptom relief, and provides all the necessary drugs and therapies, medical supplies and equipment. Typically, a family member serves as the primary caregiver and, when appropriate, helps makes decisions for the terminally ill individual. Members of the hospice team make regular visits to assess the patient and provide additional care or other services. While the range of services provided will vary depending on each individual situation and specific needs, the hospice staff is available by phone 24 hours a day/7 days a week.

  • Patients must meet tow qualifications for Hospice:
    1. Two physicians, the attending physician and the hospice medical director, must certify the patient is terminally ill, with a six-month or less life expectancy if the disease takes its normal course.
    2. The patient and/or family must be aware of the prognosis and elect comfort care, rather than active curative measure.

  • Medicare befits, and most private insurance companies, pays for hospice care as long as the patient continues to meet the criteria necessary. Patients may come on and off hospice care, and re-enroll in hospice care, as needed.

  • Hospice focuses on comfort. Dignity, and emotional support. The quality of life for the patient, but also family members and others, who are caregivers, is top priority.

  • Depending on your circumstance and stage of care, a hospice team may provide any combination of the following services:
      • Nursing - Registered nurses monitor your symptoms and medication, and help educate both you and your family.
      • Social Services – A social worker counsels and advises your family, and acts as your community advocate making sure you get the resources you need.
      • Physician Services – Your doctor approves the plan of care and works closley with your hospice team.
      • Spiritual Support and Counseling – Spiritual care is a personal process, and may include helping you explore what death means to you, resolving “unfinished business,” saying goodbye to loves ones, and performing a specific religious ceremony or ritual.
      • Home Health Aides – Home health aides provide personal care such as bathing, shaving, and nail care. Homemakers are available for light housekeeping and meal preparation.
      • Respite Care – Gives your family a break from the intensity of caregiving.
      • Bereavement Support – Bereavement is the time of morning we all experience following the loss of a loved one. The hospice care team will work with your family members to help them through the grieving process.
    Patients keep their own physician who works closely with all of the of the hospice team and medical director to carry out a plan that is right for you and your family’s needs during this time.

  • It allows people to spend their final days as comfortable as possible, surrounded by family and friends in a familiar environment. Home-based hospice gives patients and their families a greater sense of control than a hospital allows.
